Script on portal field only executes correctly for first portal row.

I have a portal with some fields in it and a script attached to one of these fields. This script checks what value has been entered into its attached field and uses that value to do a lookup of another value, and place this value in a different field. (E.g. looking up price of an item given a code)

 

However, this works for only the first portal row. Is there any caveat or unseen problem I'm running into?
