Calculating Name Fields from Individual members to Family Group

I have a membership database which has both family memberships (for mailing, voting, etc) and individuals for tracking items unique to them. I have an address table & a name table, related by a family ID. I'm now trying to calculate with a self join of the name table and formal address name (i.e. Mrs. & Mr. Jones or Dr. Dale Jones & Mrs. Jo Kurtz).

 

I keep thinking I'm almost there, but not quite. Anyone out there have an easy solution.
