check boxes in a form

I am working on a form which I want someone (Quality Assurance) rep to open in their web browser and check of some checklist items - then put their name in a box indicating the relase of a shipment.

Somewhat like specifying a button to do something with a picture (of a checkmark) but this is kind of convoluted???

 

Any suggestions on a way to do this?
